Job Summary
Provide structure and guidance to staff at all levels in the planning and implementation of the person-centered philosophy. Monitors and facilitates pre planning to ensure all policies of the life plan process are adhered to. Identifies improvements in existing planning processes to enhance innovation. Uses an innovative approach on an ongoing basis to support individuals with IDD and their care teams with an emphasis on finding new possibilities for each person supported. Utilizes a collaborative and consensus approach and is capable of leading cross functional teams in virtual and in-person forums.
In addition, this position has DEIB (Diversity, Equity, Inclusion & Belonging) responsibilities. The incumbent will support the Chief Diversity, Equity, and Inclusion Officer to build an effective DEIB strategy, creating Employee Resource Groups, preparing communications, event planning and coordination, sourcing and managing partnerships with diverse professional networks and associations.
This position supports all programs in New York and New Jersey. The Position resides in New York and operates on a hybrid schedule.
Key Tasks and Responsibilities
Advocacy and Innovation
- Tracks the pre-planning/review process and formulates a strategic plan that reflects each individual’s progress and facilitates changes needed when identified.
- Ensures written plans reflects person centered ideology. Goals are formulated in Personal Outcome Language
- Conducts Personal Outcome Measure Interviews as a foundation in the creation of valued outcomes and staff supports.
- Coordinates with the treatment team, families, advocates, and individuals to ensure that concerns are addressed during the meeting and plan development process.
- Reviews billing data for accuracy and compliance with policies.
- Updates job knowledge by attending staff meetings; participating in educational opportunities; reading professional publications; maintaining personal networks.
- Inputs, reviews and approves habilitation plans in Therap.
- Readily shares information with others in compliance with HIPPA policies and procedures.
- Provides management staff with guidance in training staff on the program plans with an emphasis on the person-centered philosophy.
- Assesses the Service Recipient needs and desires as a way to help the team formulate the most appropriate plan for each person.
Diversity, Equity, Inclusion & Belonging
- Assist in the development and administration of DEIB trainings, workshops, and special events.
- Training the workforce on DEIB topics.
- Creating communication strategies via email, newsletters, and announcements keeping the workforce informed of DEIB updates.
- Assist with identifying and developing relationships with external sources and peer organizations to enhance DEIB efforts.
- Keeping updated with current DEIB trends and practices.
- Perform other related tasks or projects as assigned
Education and Experience Requirements
- Bachelor’s Degree
- Demonstrated commitment to valuing diversity and contributing to an inclusive working and learning environment.
- 2+ years of experience in diversity related programs
Salary Range
$65,000 – $75,000
