TBI Service Coordinator 

Duties include but are not limited: Seeks full-time Service Coordinator(s) to serve individuals with traumatic brain injury

Location: In all boros including Westchester and Long Island. Candidate must be able to commute/drive

Requirements: Candidates must possess a minimum of a BA Degree in social work or related field with two years experience or Master Degree with one year experience in providing case management/service coordination services to populations with disabilities and valid NYS driver’s license. Salary commensurate with credentials and experience

Senior Direct Care/Medical Coordinator

 

Duties include but are not limited: In addition to carrying out the responsibilities of a Direct Support professional, candidate is responsible for scheduling, tracking, documenting medical appointments and lab tests for developmentally disabled individuals within a residential/OMRDD facility. The right candidate will also be responsible for taking consumers to appointments and advocating for them.

 

Location: Positions available in the Bronx and Queens.

 

Requirements: Must have a valid NYS Driver’s license for at least 1 1/2s year, excellent verbal and written skills, experience working in OMRDD, AA degree preferred or High School with a 2 year of experience.

Residence Manager

Program Instructor

Hours of position M-F 8AM-4PM. Experience with writing and implementing both IHP’s and ELP’s. Independent Living Skills Trainer – P/T or F/T

Progressive non-profit organization seeks P/T & F/T  ILST trainers.  Must have experience in assessing/implementing goals and training individuals with Traumatic Brain Injury. AA/BA a+. Program Supervisor- ILST Assistant Supervisor F/T

Responsibilities include assisting with the daily supervision and management of the TBI-ILST program.  Complete bi-weekly payroll, review staff notes, goal plans, and semi-annual reports, scheduling, assist in the monitoring and development of staff training, update existing tracking systems, follow-up with staff regarding required paperwork, conduct internal audits of books, conduct field visits as required. Position based out of L.I. office, Valid NYS Driver, Must travel inter-office; all 5 boroughs are including L.I. and WC
